General Liability Waiver & Release of Claims

Liability Waiver, Assumption of Risk & Indemnity Agreement

1. Voluntary Participation & Assumption of RiskI understand that participation in any sports event/activity, under the Community Sports Events, involves inherent risks, including but not limited to: physical injury, slips, falls, collisions, dehydration, heat-related illness, equipment-related injury, and other risks that may arise from my own actions or those of others.I voluntarily assume all risks associated with participating in this event.
2. Release of Liability
   I hereby release and discharge the Community Sports Events, Clubs, event organizers, sponsors, partners, referees, staff, and volunteers from all liability, claims, demands, or causes of action relating to:
   • Personal injury or illness
   • Property loss or damage
   • Accidents during participation or use of facilities
   • Weather conditions, environmental factors, or equipment failure
   • Exposure to communicable diseases

   This release applies to all claims arising from negligence, except gross negligence or willful misconduct.

3. Medical Authorization
   I consent to first aid, medical assessment, or emergency treatment if required during the event. I understand that any medical expenses incurred are my sole responsibility.
4. Indemnity
   I agree to indemnify and hold harmless all parties listed above from any claims arising from my conduct, rule violations, or misuse of facilities or equipment.
5. Minors (If Applicable)
   If registering a minor, I affirm that I am the parent/legal guardian and accept full responsibility for the minor’s participation and all terms of this waiver.
6. Media Consent
   I grant permission for the use of photos, videos, or recordings taken during the event for promotional and documentation purposes without compensation.
7. Rules & Conduct
   I agree to abide by all event rules, safety instructions, and directions from referees and staff. Failure to comply may result in removal from the event without refund or compensation.
8. Event Changes or Cancellation
   I acknowledge that the organizer may modify schedules, delay games, or cancel the event due to weather, safety risks, or unforeseen circumstances, without obligation for compensation.
